Job Description: Shared Living Specialist- PT/FT
Responsibilities:
- Ensuring all clients are treated with respect and dignity, recognizing individual clients needs and encouraging independence/support where needed
- Performing walk through inspections of potential SLP placements and Respite providers
- Offering training and guidance to new SLP applicants
- Checking in with SLP applicants on a quarterly basis to keep waitlist updated
- Coordinating with SLP and Human Resources for proper intake of applicants
- Ensuring all background checks and training is completed for back up staffing/follow up with HR
- Conducting Therap training with new Shared Living Providers and their support staff/back-up staff.
- Participation in the referral/intake process
- Monthly review of program data for Quality Assurance
- Conducting, completing, and submitting Quality Assurance “Monthly Consumer Reports” within the SLP home
- Assisting with oversight of all the client’s medical needs, insurance, payee needs, service needs, modification to the home, management of medications, etc.
- Offer 24/7 guidance, crisis management to SLP as needed
Qualifications:
- 1-3 years of experience working as direct-line staff with persons with developmental disabilities.
- Service delivery systems for individuals with developmental disabilities.
- Experience in Word, Excel, Outlook and Therap.
- Ability to connect and build effective working relationships with the clients, staff, and Shared Living Contractors
- Communicate effectively, both written, verbally, and interpersonally.
- Work in a complex and nontraditional human service setting with a diverse population.
- Outstanding organization skills
- Great time-management skills
- Takes initiative
- Experience in multi-tasking
Requirements:
- A degree in a related field, or course study working toward a degree preferred or equivalent work experience
- Valid Driver’s License
- Current Vehicle Insurance (upon request)
- MANDT, CPR, First Aid, Medication Aid training (preferred but will train if necessary)
